Feeling sad during a difficult period is human. Depression, however, can affect mood, interest, sleep, energy and everyday functioning for a sustained period. Understanding the differences can make it easier to recognize when an assessment may help.

How is depression different from sadness?

Sadness may follow grief, disappointment or stress, and people can still experience moments of relief or pleasure. Depression can involve persistent low mood or loss of interest along with other symptoms that affect how a person thinks, feels and functions.

A stressful event does not rule out depression. The presence, duration and effects of symptoms are more useful than judging whether someone has a good reason to feel down.

Symptoms beyond feeling unhappy

Depression may involve difficulty concentrating, low energy, changes in appetite or sleep, irritability, feelings of guilt or worthlessness and loss of enjoyment. Some people continue working while struggling, so appearances can be misleading.

A major depressive episode generally involves symptoms occurring most of the day, nearly every day, for at least two weeks, but diagnosis requires professional assessment.

Could other factors explain the changes?

Medical conditions, certain medications, sleep problems, substance use, grief and other mental health conditions may contribute to mood changes. Relevant history and medical evaluation can help differentiate possible causes.

Telling a clinician about periods of unusually high energy, less need for sleep or other significant changes is important because this information may affect assessment and treatment decisions.

What does an evaluation involve?

A psychiatric evaluation may include current mood symptoms, their timing, family history, medications, health conditions and their effect on daily activities. The clinician may ask questions about safety and coping, as well as your own goals.

What treatment conversations may follow?

Depending on assessment and clinical scope, options may include psychotherapy, medication, lifestyle supports or referral to other services. A clinician should discuss benefits, risks and alternatives rather than promising one approach will work for everyone.

If mood changes are persistent or concerning, contact the practice to ask about availability.

Can grief and depression occur together?

Yes. Grief and depression can coexist, and a clinician considers the nature of each experience.
